Iran's IRGC Navy said a shuttle tanker laden with 750,000 barrels of crude oil — described as the third oil tanker to hit mines on a prohibited route — struck a naval mine in the southern Strait of Hormuz, triggering an explosion and fierce fire (IRNA English restatement 12 October 02:10 UTC, with native video). Earlier on 11 October, IRNA, Press TV and Al Mayadeen carried the same IRGC claim of a violating crude oil supertanker mine strike with an engine-room fire. On the afternoon of 12 October, Press TV released further IRGC Navy footage it framed as the third Hormuz supertanker hit over two days and said the tanker had switched off its navigation system, ignored IRGC Navy warnings and attempted to leave the strait 'like the two previous vessels'; Osint613 relayed the same IRGC/Fars attribution. The Maritime Executive reported on 12 October that the stern visible in the unverified IRGC footage of the tanker said to have been 'ignoring warnings and switching off its navigation system' displays the name of the UAE-owned VLCC Al Maqam (IMO 9732577); on 14 October FaytuksNetwork, citing Seatrade Maritime, likewise identified the fire-damaged tanker in Tasnim footage as ADNOC's VLCC Al Maqam. Neither the IRGC nor UKMTO named the vessel, and it remains unclear whether it matches the 750,000-barrel description or any UKMTO-reported 11 October incident.

Evidence: IRGC Navy: violating supertanker hit naval mine south of Hormuz on Sunday; engine room ablaze, IRGC says a second/further supertanker struck a naval mine in the Strait of Hormuz, IRGC: explosion/fire after naval mine south of Hormuz; third vessel since yesterday on prohibited route, IRGC Navy: shuttle tanker (third) with 750,000 barrels crude hit sea mine on prohibited route — explosion and fire, IRGC Navy releases footage of the third supertanker hit in Hormuz over the past two days, IRGC: tanker disabled navigation, ignored Navy warnings before attempting to leave the strait, IRGC/Fars: tanker hit after disabling navigation and ignoring IRGC Navy warnings
IRGC claim via state media (IRNA/Press TV/Mayadeen 11 Oct; IRNA restatement 12 Oct with cargo tonnage; Press TV/Fars 12 Oct afternoon follow-up on navigation disablement and new footage). No UKMTO numbered product names this vessel; maritime media (Maritime Executive, Seatrade via FaytuksNetwork) read the name Al Maqam on the stern in the IRGC footage, not independently verified; ADNOC has not commented.
